Entry form

Participants must register by filling in an entry form and send it to the local campaign centre no later than the quit day. The form should contain the following core information: name, date of birth, sex, address, telephone number, number of smoking times per day (cigarettes, cigars and pipefulls smoked per day), previous attempts to quit (never, 1-2 times, 3 times or more), years of smoking, and name and address of a witness. Additional questions can be included according to local needs.

The witness must be able to testify that the participant was eligible to the competition (=daily smoker) and to testify the smoking history of the participant during the four week competition period.

In the form the participant should certify that he/she will comply with the rules, tries to abstain from smoking during the contest period and promises to tell the truth, if asked.
